Pigmented Lesions
Solar lentigines (age spots, sun spots), freckles, ephelides, and diffuse mottled pigmentation from chronic sun exposure. IPL energy is absorbed by epidermal and superficial dermal melanin, causing selective photothermolysis of pigmented cells. Lesions typically darken and crust before shedding within 7–14 days.
Vascular Lesions
Facial telangiectasia, rosacea, diffuse facial erythema, spider veins, and port wine stains. Oxyhaemoglobin absorbs IPL energy, causing thermal coagulation of the vessel wall and subsequent clearance. The 515nm and 580nm filters are most effective for superficial vascular targets.
Rosacea & Facial Redness
Persistent background erythema, flushing, and telangiectasia associated with rosacea. Multiple sessions with lower fluence settings and the 515nm or 580nm filter progressively reduce background redness and visible vessels. Combination with anti-rosacea skincare (iS Clinical) enhances and prolongs outcomes.
Sun Damage & Photorejuvenation
Broad-spectrum treatment of photodamaged skin — simultaneously addressing pigmentation irregularity, vascular changes, and collagen stimulation for overall skin quality improvement. A foundational treatment for patients presenting with mixed sun damage and early ageing changes.
Melasma (Adjunctive)
IPL can be used as an adjunctive treatment for melasma with appropriate patient selection and filter choice, typically combined with a comprehensive topical regimen (hydroquinone, retinoids, SPF). Caution is required with melasma — incorrect settings can worsen the condition. Restricted to Fitzpatrick I–III for this indication.
Body Areas
Lumecca Peak can be applied beyond the face to the décolletage, neck, hands, and arms for consistent photorejuvenation across all chronically sun-exposed areas. Body treatments typically require slightly higher fluence settings due to thicker skin and lower density of target chromophores.

Filter selection is the primary determinant of target chromophore — use shorter wavelength filters (515nm) for superficial pigment and vessels on lighter skin, and longer wavelength filters (580nm, 640nm) for deeper targets and darker skin types. Filter Selection Guide
The 515nm filter passes wavelengths from 515nm upward — optimal for superficial epidermal melanin and superficial vessels on lighter skin types (Fitzpatrick I–II). The 580nm filter provides a longer wavelength cut-off suited to slightly deeper targets and Fitzpatrick III–IV skin. The 640nm filter is reserved for deeper dermal pigment, Fitzpatrick IV, or as the baseline filter for hair reduction applications. Always use a longer wavelength filter as skin type increases to reduce the risk of epidermal overheating and post-inflammatory hyperpigmentation.
Important Clinical Notes
Apply chilled ultrasound gel to the treatment area before each pass — this provides optical coupling and surface cooling. Do not treat tanned skin; request patients avoid sun exposure and apply SPF daily for a minimum of 2–4 weeks before treatment. Post-treatment: apply cooling gel or cold pack for 5–10 minutes. Patients should avoid sun exposure and apply SPF50+ for a minimum of 4 weeks post-treatment. Pigmented lesions will darken before shedding — advise patients of this expected response to prevent concern.

Filter selection is based on the patient's skin type and the target chromophore. For lighter skin types (Fitzpatrick I–II) with superficial pigment or vascular lesions, the 515nm filter delivers the highest melanin and oxyhaemoglobin absorption. For Fitzpatrick III–IV, switch to the 580nm filter to allow the longer wavelength to bypass epidermal melanin more safely and target deeper chromophores. The 640nm filter is reserved for deeper dermal targets or as the standard filter for darker skin types. As a general rule: the darker the skin type, the longer the wavelength filter required.
Patients describe the sensation as a brief, sharp snap — similar to a rubber band flicking the skin — with a transient heat sensation. The treatment is generally well-tolerated without anaesthesia; for sensitive patients or higher fluence settings, topical anaesthetic can be applied. Immediately post-treatment, erythema and mild oedema are expected and typically resolve within a few hours. Pigmented lesions will darken over 24–48 hours before gradually flaking off over 7–14 days. Social downtime is typically 1–3 days.
Lumecca Peak is recommended for Fitzpatrick skin types I–IV. For Fitzpatrick IV, use the 580nm or 640nm filter with conservative fluence settings and always perform a patch test before treating. Fitzpatrick V–VI skin types are not recommended candidates for IPL treatment due to the high risk of epidermal overheating, burns, and post-inflammatory hyperpigmentation from the high melanin concentration in the epidermis competing with the target chromophore. For darker skin patients requiring pigment or vascular treatment, alternative modalities such as Nd:YAG laser are generally preferred.
Most patients require 3–5 sessions spaced 3–4 weeks apart for optimal pigment and vascular clearance. Discrete lesions such as solar lentigines may clear in 1–2 sessions; diffuse photorejuvenation and rosacea typically require the full course. Results continue to improve for 4–8 weeks after the final session as the skin regenerates. Maintenance treatments every 6–12 months are recommended for patients with ongoing sun exposure or rosacea predisposition to sustain results.
